Job Description

Posts available: 3.
Contract/Term: 1 year fixed-term contract with possible extension, subject to funding.
Salary: £25,000 per annum plus pension after qualifying period.
Hours per week: Full Time, 35 hours per week between 8 AM – 8 PM and may include occasional weekends (rota basis/don’t be put off by this!).
Reporting to: Health Junction CIC & PCN Leads.
Location: SWAGGA Primary Care Network – South Liverpool.

Role Details

An exciting opportunity has arisen for a social prescribing link worker to join our growing team of link workers based in SWAGGA Primary Care Network, South Liverpool.

You might be looking to change your role and make a real difference in patients’ lives by supporting and signposting patients through referrals of non-medical pathways to support and link patients to resources within the local community. You will become their expert of local resources that patients may need support to access.
